Discharge & Patient Flow Coordinator

Medway Mental Health NHS Trust is seeking an enthusiastic Assistant Discharge Coordinator to join the Discharge team at Littlebrook Hospital. You will help coordinate early and complex discharges, work with the MDT, and support patients and carers to address discharge challenges while optimise bed capacity.

Applicants should have strong communication skills and a proactive approach, with eligibility to drive a UK vehicle for business use.
