Discharge Social Worker: Adult Care (Hybrid)

Liquid Personnel is recruiting a Social Worker to join the Adult Hospital Discharge Team in St Helens. The role combines office, ward-based and remote work on a rota, supporting timely discharges and safe transitions from hospital to home.

You will carry out assessments, arrange care packages, and collaborate with health and social care professionals to ensure person-centred discharge outcomes. A post-qualification background in discharge planning is preferred.
