Family & Community Outreach Specialist

Durham County Council is seeking enthusiastic practitioners to join Family Hub Teams supporting children and families in community settings. The Outreach Worker will engage with children, young people and families aged 0–19 (or 25 with SEND), assess needs and deliver group-based interventions in community venues and homes.

The role requires a Level 3 qualification, experience with children and families, multi‑agency collaboration, group activity delivery, and strong communication skills.
