About the Role

Our client is seeking a dedicated Family Support Practitioner to join their team in Derby . This role is crucial in providing essential support to families, helping them to overcome challenges and build resilience. You will work collaboratively with families, offering practical guidance, emotional support, and access to resources to improve outcomes for children and parents. This position involves a blend of in-person visits to families' homes and community settings, alongside remote communication and administrative tasks, offering a varied and impactful role within the community.

Key Responsibilities

• Conduct assessments of family needs and develop tailored support plans.
• Provide direct support and guidance to families facing various challenges, including parenting, finances, and relationships.
• Facilitate access to external services and resources, such as housing, education, and health services.
• Work with families to develop coping strategies and improve communication and problem-solving skills.
• Maintain accurate and confidential records of all interactions and progress made.
• Liaise with other professionals, including social workers, health visitors, and educators, to ensure a coordinated approach.

Requirements

• Experience in family support , social work, childcare, or a related field.
• A strong understanding of child development, family dynamics, and safeguarding principles.
• Excellent communication, empathy, and active listening skills.
• Ability to build rapport and trust with families from diverse backgrounds.
• Proficiency in using IT systems for case management and reporting.
