Family Support Practitioner

About the Role

Our client is looking for a dedicated Family Support Practitioner to join their vital services in Liverpool . This role is dedicated to strengthening families and improving outcomes for children by providing targeted support, guidance, and intervention. You will work collaboratively with families to address challenges, build on their strengths, and connect them with essential resources within the community. Key Responsibilities

• Conduct comprehensive assessments of family needs, identifying strengths, risks, and areas for development.
• Develop and implement effective family support plans, setting clear goals and strategies.
• Provide practical and emotional support to families, including parenting advice and behaviour management strategies.
• Facilitate access to relevant external agencies and services, such as health, education, and housing support.
• Monitor progress and review support plans regularly, adapting interventions as needed.
• Maintain accurate and confidential case records in accordance with organisational policies and procedures.

Requirements

• Relevant qualification in social work, youth work, community development, or a related field.
• Proven experience working directly with families, ideally in a support or intervention capacity.
• Strong understanding of child development, safeguarding principles, and family dynamics.
• Excellent communication, active listening, and negotiation skills.
• Ability to build rapport and establish trusting relationships with diverse client groups.
• Proficiency in IT systems for case management and reporting, specifically within the Liverpool area.
