Frailty Lead ACP | Advanced Clinical Practitioner

Buckinghamshire Healthcare NHS Trust seeks a Lead Frailty Advanced Care Practitioner to head the expanding ACP team and provide autonomous clinical support in frailty assessments, diagnoses and discharge decisions within agreed protocols.

The post holder will rotate across Stoke Mandeville Hospital and community sites at Amersham and Buckingham, supporting 24/7 frailty care with dedicated leadership and education time to develop the service.
