GP Clinical Pharmacist – Medicines Optimisation Lead

K & W Healthcare is seeking an experienced Clinical Pharmacist with General Practice experience to join our Brent-based GP network. You will collaborate with GPs, nurses and the multidisciplinary team to optimise medicines and improve prescribing within primary care.

The role involves structured medication reviews, adherence support and applying NICE guidelines, with flexible hours and a self-employed contract across multiple Brent practices. Experience with GP settings is essential.
