Home-Based Stroke Rehab Support Worker

Nottinghamshire Healthcare NHS Foundation Trust is seeking a compassionate Rehabilitation Support Worker to join the Community Stroke Team across South Nottinghamshire. You will deliver therapy-led activities and support patients in their homes, working with clinicians to achieve rehabilitation goals.

You will be part of a multidisciplinary team, assessing needs, planning treatment, and providing encouragement to improve mobility and daily living skills.
