Hospital Discharge & Care Transitions Specialist

The Cumberland is seeking a Trusted Assessor to support hospital discharge processes, coordinating access to post-discharge services and ensuring safe, timely transitions for adults.

You will work with Case Managers, Discharge Navigators and a multidisciplinary team to triage and route individuals to appropriate community or in-hospital care, in a role that involves regulated activity with adults.
