Hospital Social Worker II – Elder Care, Strengths-Based

Warwickshire County Council is seeking a Level 2 Social Worker to join the Hospital Social Care Team based at Hospital of George Eliot Hospital Nuneaton and Rugby St Cross Hospital. You will manage a caseload of older adults in hospital settings, complete robust assessments, and develop care plans while liaising with health partners and carers.

The role requires a professional Social Work qualification and HCPC registration, with a 5-day/7 schedule and occasional weekend work.
