Impactful Community Health Link Worker

Phoenix Health PCN is seeking a part-time Social Prescriber Link Worker to support people with long-term conditions, frailty and social needs. You will work within a multidisciplinary team across Buckinghamshire to improve wellbeing and independence through personalised support and community-based solutions.

The role involves coordination with GP practices, planning care with other professionals, and providing information about services to promote health and prevent unnecessary hospital visits.
