About the Role

Key Responsibilities

• Provide remote guidance and support to individuals to help them sustain independent living.
• Assist clients in identifying and accessing appropriate community resources, benefits, and services.
• Conduct virtual check-ins to assess client well-being and address any emerging needs.
• Facilitate connections between clients and relevant support networks and service providers digitally.
• Educate clients on managing daily tasks, personal safety, and utilizing technology for independence.
• Maintain accurate and confidential client records and progress notes in the designated online system.

Requirements

• Previous experience in a care, support, social work, or related role.
• Excellent remote communication skills, including active listening and empathy via digital platforms.
• Proficiency with virtual collaboration tools, and digital record-keeping software.
• Strong problem-solving capabilities and a client-centered approach.
• Ability to work autonomously and manage time effectively in a remote work setting.
• Understanding of the challenges faced by individuals needing support for independent living.
