What we ask from you?

• Working in a multi-disciplinary team alongside Clinical and Operational Leadership to ensure high quality care delivery to service users.
• Provide specialist assessment and care planning in conjunction with service users and carers.
• Evaluate treatment plans and undertake discharge planning and promote self care.
• Be responsible for an agreed caseload.
• Co-ordinate the work of the Team.
• Support development of skills and competencies in others.

Essential Qualifications and training

• Nursing Registration with the NMC
• Degree in nursing or equivalent
• Mentorship/Assessor Qualification
• Essential experience
• Experience in leadership
• MDT working
• Caseload Management
• Clinical skills relevant to community nursing - fully completed skills passport
