Lead Care Specialist - Dementia & Senior Care

HC-One is seeking a committed Senior Care Assistant to lead a care team in delivering personalised dementia, nursing and residential care. You will support residents with dignity and independence, while building strong relationships with families and staff.

You will ensure accurate records, plan and implement care that meets individual needs, and work closely with the Registered Nurse and Care Manager to provide holistic support, safeguarding, and meaningful activities across shifts.
