Key Responsibilities
- Develop, implement, and review person-centred support plans in collaboration with individuals and their families/carers.
- Coordinate and facilitate a range of support services, including healthcare, social activities, and personal care.
- Act as a key point of contact and advocate for individuals, ensuring their views and choices are respected.
- Liaise with healthcare professionals, social services, and community groups to ensure seamless support delivery.
- Monitor the effectiveness of support arrangements and make necessary adjustments to meet evolving needs.
- Maintain accurate and up-to-date records, adhering to confidentiality and data protection policies.
Requirements
- Experience working with individuals with learning disabilities is essential.
- A relevant qualification in health and social care, such as an NVQ Level 3 or equivalent, is highly desirable.
- Excellent communication, interpersonal, and advocacy skills.
- Strong organisational and planning abilities, with the capacity to manage multiple cases remotely.
