About the Role
Our client is seeking dedicated Learning Disability Support Workers to join their supportive team in Portsmouth . This essential role involves providing person-centred care and support to individuals with learning disabilities, enabling them to lead independent, meaningful, and fulfilling lives. You will assist clients with various aspects of their daily lives, encourage community involvement, and promote their rights and choices. Key Responsibilities
- Provide direct support to individuals with learning disabilities, assisting with daily living tasks, personal care, and independence skills.
- Develop and implement person-centred support plans in collaboration with the individual, their family, and other professionals.
- Facilitate social inclusion and encourage participation in community activities, hobbies, and education.
- Support individuals in managing their health needs, including attending appointments and administering medication as directed.
- Promote individuals' rights, choices, and independence at all times.
- Maintain accurate and confidential records of support provided and report any concerns to the appropriate supervisor.
Requirements
- Experience supporting individuals with learning disabilities is highly desirable.
- Excellent communication, empathy, and patience.
- Ability to work effectively within a team and on your own initiative.
- Understanding of person-centred planning and safeguarding principles.
- Willingness to undertake relevant training and professional development.
