About the Role

Key Responsibilities

• Provide direct support to individuals with learning disabilities, assisting with daily living tasks, personal care, and independence skills.
• Develop and implement person-centred support plans in collaboration with the individual, their family, and other professionals.
• Facilitate social inclusion and encourage participation in community activities, hobbies, and education.
• Support individuals in managing their health needs, including attending appointments and administering medication as directed.
• Promote individuals' rights, choices, and independence at all times.
• Maintain accurate and confidential records of support provided and report any concerns to the appropriate supervisor.

Requirements

• Experience supporting individuals with learning disabilities is highly desirable.
• Excellent communication, empathy, and patience.
• Ability to work effectively within a team and on your own initiative.
• Understanding of person-centred planning and safeguarding principles.
• Willingness to undertake relevant training and professional development.
