Mental Health Practitioner - Later Life & Frailty (Band 6)

Surrey and Borders Partnership NHS Foundation Trust is seeking a Band 6 Mental Health Practitioner to join the Woking Community Mental Health Team for Older People. You will deliver evidence-based, recovery-focused care to adults with complex mental health needs to help them live independently in the community.

The post requires NMC registration as a Mental Health Nurse, mentorship and supervision training, and Practice Assessor qualification.
