Job Description
An exciting opportunity has arisen for a highlymotivated and enthusiastic Mental Health Practitioner to join the CommunityMental Health and Memory Service.
Main duties of the job
Ourideal candidate would be a committed team player as well as being able to workindependently, demonstrating initiative and be passionate in promoting goodquality care to maximise independence. Within this role you will be responsiblefor assessing, developing, implementing and evaluating care plans using aperson-centred approach. Within this role you will gain training, support, andguidance to ensure you are working collaboratively with external agencies to providethe best possible care for our service users.
Thisrole requires someone who can manage in a fast-paced environment completingassessments to support diagnosis. The day-to-day work - for this role will beled by the needs of the service users and the service which will requireeffective time management and prioritisation skills as well as effectivecommunication and autonomous working.
Allofour staff are an integral part of the multidisciplinary team within mentalhealth services.

Barriers to understanding include: working with service-users who couldbe hostile or highly emotional, who have complex needs and may be vulnerable.

2.Knowledge,training and experience
Thepost-holder will hold current registration on part 3 or 13 of the NMC registeror hold a Registered Occupational Therapist, Social Worker, Mental Nursing(RMN) certificate.
Havepost registration experience and / or training, relating to older people andmental health and apply this knowledge when delivering care to service-users.
Provideadvice and support to other agencies around issues relating to mental health.
3.Analytical &judgement skills
Undersupervision make decisions about the range of options for treatment and careplans based on judgement of service-users need.
Conductcomprehensive assessments of both service users and carers in line with teamoperational policies.
4.Planning &organisational skills
Planand co-ordinate care plan review meetings with colleagues in the team and withstaff in other teams and agencies, ensuring that review meetings take place atthe appropriate time.
Co-ordinatetransfer or referral of service-users to other teams or services whererequired.
Undersupervision, prioritise, plan and co-ordinate own workload and activities inline with the changing needs of the team and service. This includes planningand monitoring client medication needs, making appointments for service-users,conducting assessments, managing emergencies and enquiries, responding tocrisis work within individual caseload, ensuring patient records are up to dateand supporting colleagues on a daily basis.
Havethe ability to work independently and to be able to resolve complexprofessional problems using supervision appropriately.
Crisisresponse to other staffs service-users, in the event of key workers absenceand delegation from manager.
Participatein caseload management and ensure relevant information is available for this totake place.
5.Physical skills
Betrained in control and restraint and breakaway techniques and undertake regularupdates.
6.Responsibilitiesfor patient/client care
Developprogrammes of care for service-users and act as care co-ordinator forservice-users, ensuring links with Adult Social Care.
Assessand review service-users clinical risk management plan using the DICES riskassessment.
Assesscare needs of service-users and carers to develop and implement appropriatecare plans.
Providespecialist clinical advice to service-users, their carers, relatives and otherprofessionals on needs relating their difficulties and mental health. Thisincludes a range of health education and advice.
Initiateindividual carer support where required.Set objectives, monitor and audit effectiveness.
Participatein clinical governance activities.
7.Responsibilitiesfor policy & service development implementation
Adhereto all Navigo policies and legislation relating to the role.
Contributeto the evaluation and development of the service.
Commenton policies, procedures and developments where required.

Accuratelydocument episodes and information relating to client care in patient notes,using agreed methods of documentation. This will be done in accordance with Navigopolicies, professional guidelines and data protection legislation.
Recordpersonally generated information as required by the team or service, asdirected by the Clinical team leaders / Team Manager.
Provideoral / written clinical reports as required.
Managehighly confidential information in respect of members of the public inaccordance with local and national policy.
11.Responsibilitiesfor research and development
Participatein surveys or audits or research in relation to own work, as required by theteam or service.
Beaware of up to date relevant published research, benchmarking and innovativepractice in other areas and ensure own practice is evidence based.

Delivercare for service-users and carers, working within professional codes ofpractice, organisational policies, local protocols and national legislation.
Havethe ability to work independently, with peer support and supervision whererequired, to deliver care for service-users / carers and resolve complexprofessional problems.

Workin a clinic / office based environment.
Regularlydrive to visit service-users in their own homes or in a community setting(provision to make reasonable adjustments under the Disability DiscriminationAct applies).

Concentrationis frequently required in order to carry out client assessments and write uppatient episodes in notes on the majority of working days.
Workis carried out in an unpredictable environment. This includes, for example,working with service-users who display unpredictable behaviour, responding toemergency telephone calls, responding to immediate risks to service-users andothers, and to deal changes and queries from service-users carers and otherprofessionals and agencies.

Workdirectly with difficult family situations and circumstances. For example,communication with service-users, families and other agencies around receivinga diagnosis of dementia, adult protection, suicide and overdose, and othermental health issues.
Workwith service-users who display challenging and potentially verbally and / orphysically aggressive behaviour.

Someexposure to unpleasant working condition, much of the work is carried out inthe community
