Mental Health Social Worker - Primary Care Network (Hybrid)

Blackburn with Darwen Council is seeking a Mental Health Social Worker to join the Primary Care Network team in the Daisyfield Mill area. You will work in multi-disciplinary teams across neighbourhood hubs to prevent crises and promote integrated community wellbeing for residents.
