This is an exciting opportunity for an experienced and qualified Occupational Therapist to join the Reablement Team in Weston-super-Mare, Somerset. This interim position offers full-time work, with 37 hours per week from 09:00 to 17:00, focusing on supporting discharge-to-assess caseloads in Adult Social Care. What you will do:
- – Support discharge-to-assess caseloads within Adult Social Care, ensuring smooth transitions for clients.
- – Complete moving and handling risk assessments tailored to individual client needs.
- – Undertake Mental Capacity Assessments and make informed Best Interests decisions.
- – Carry out case management activities to effectively meet client goals.
- – Complete care assessments to determine the best support plans.
- – Undertake Occupational Therapy goal setting to enhance client wellbeing.
- – Assess and recommend necessary equipment and adaptations to improve client independence.
Additional Information:
- Temporary assignment, offering a short-term but impactful opportunity.
- IR35 status: In scope, meaning employment rights and obligations will be actively considered.
- Enhanced DBS required for safeguarding.
- Community visits are required, with travel supported by mileage reimbursement.
- Ideally, candidates should be local to the North Somerset area, bringing valuable local knowledge to the team.
- A car driver is required to navigate community visits effectively.
Requirements:
- You must be a qualified Occupational Therapist with relevant experience in Adult Social Care.
- Experience supporting discharge-to-assess pathways is essential.
- Competency in completing moving and handling assessments, along with Mental Capacity Assessments and Best Interests decisions, is required.
- An Enhanced DBS clearance is necessary.
- The ability to travel throughout North Somerset is essential.
- Local knowledge of the North Somerset area.
- Experience working alongside Reablement Services.
