Street Outreach Navigator – Rough Sleeping

Location: Northampton Hours: 37.5 Contract: 3 months Pay: £34 Umbrella

• Car driver required, travel across Northampton.

We are looking for a Street Outreach Navigator to join a dedicated Rough Sleeping Services team working across North Northamptonshire.

This is a rewarding opportunity for someone who can build trust with people experiencing rough sleeping, understand their individual circumstances and provide practical, hands-on support to help them access accommodation, healthcare and other essential services.

What you'll be doing

As a Street Outreach Navigator, you will:

Carry out regular outreach across known rough sleeping locations and respond to reports of new rough sleepers.

*

Build positive, trusting relationships with individuals who may have complex or multiple support needs.

*

Provide practical and emotional support to help people move away from rough sleeping.

*

Complete assessments, risk assessments and personalised support plans.

*

Help individuals access accommodation, benefits, healthcare, mental health services and substance misuse support.

*

Support people with budgeting, debt, finances and income maximisation.

*

Work with individuals to identify their goals around training, employment, community involvement and personal development.

*

Work closely with housing, social care, health, voluntary and community organisations

*

Participate in multi-agency meetings and case conferences for individuals with complex needs.

*

Work with hospitals, prisons, landlords and supported accommodation providers to help prevent homelessness.

*

Support individuals moving from temporary accommodation into longer-term housing.

*

Support rough sleeping initiatives and emergency responses during periods of severe weather.

About you

We're looking for someone who is compassionate, resilient and motivated to make a difference.

You may have experience in:

Supporting vulnerable adults
