Outreach Support Worker

About the Role

Our client is seeking a proactive and resourceful Outreach Support Worker to provide essential services to vulnerable individuals within the Reading area, primarily operating through a remote and flexible model. This role is dedicated to supporting clients facing challenges such as homelessness, substance misuse, or long-term unemployment, helping them to access resources and achieve their personal goals. You will empower clients through guidance, advocacy, and connecting them with vital community services, fostering independence and recovery. This position offers the unique advantage of remote work flexibility, allowing for efficient management of client support across Berkshire whilst maintaining crucial community connections.

Key Responsibilities

• Engage with clients remotely, establishing trust and rapport to understand their needs.
• Provide guidance and support on accessing housing, employment, healthcare, and other essential services.
• Assist clients in developing practical life skills and personal development plans.
• Act as an advocate for clients, representing their needs to relevant agencies and support networks.
• Maintain accurate case notes and progress reports using digital platforms.
• Collaborate with internal teams and external stakeholders to ensure comprehensive client support.

Requirements

• Previous experience in social care, outreach work, or a related field.
• Strong understanding of the challenges faced by vulnerable populations.
• Excellent communication, active listening, and problem-solving skills.
• Proficiency in using digital tools for communication, case management, and reporting.
• Ability to work independently and manage a caseload effectively from a remote location.
