About the Role

Our client is seeking an enthusiastic and resilient Outreach Support Worker to join their team serving the Portsmouth area. This dynamic role involves providing essential support to individuals in their own homes and within the community, empowering them to live independently and achieve their personal goals. Key Responsibilities

• Engage with clients in their homes and community settings, building trusting relationships.
• Provide practical assistance with daily living, including support with budgeting, appointments, and accessing local services.
• Develop and implement individual support plans in consultation with clients and relevant agencies.
• Offer emotional support and encouragement, promoting clients' independence and self-esteem.
• Maintain accurate and confidential records of client progress, activities, and any concerns, using our client's digital systems.
• Collaborate with other professionals, such as social workers, healthcare providers, and housing support officers.

Requirements

• Previous experience in a support worker or similar role is advantageous.
• Excellent communication and listening skills, with empathy and patience.
• Ability to work independently and manage time effectively.
• Good IT skills for remote record-keeping and communication.
• A commitment to safeguarding and promoting the welfare of vulnerable adults.
• Must hold a full UK driving license and have access to a vehicle for community-based work in and around Portsmouth .
