Outreach Support Worker

About the Role

Our client is seeking a compassionate and reliable Outreach Support Worker to join their dedicated team in Stoke-on-Trent . This role focuses on providing essential support to vulnerable individuals within their own homes and the wider community, promoting independence and well-being. You will work on a hybrid basis, combining practical, face-to-face support with remote client check-ins and administrative tasks. This position offers a chance to build meaningful relationships with clients, understanding their needs and providing tailored assistance to help them live fulfilling lives, while contributing to a responsive and adaptable support network.

Key Responsibilities

• Provide practical assistance and emotional support to clients in their homes and community settings.
• Assist clients with daily living tasks, such as personal care, meal preparation, and medication prompts.
• Support clients in accessing community resources, appointments, and social activities.
• Conduct regular check-ins with clients via phone or video call, monitoring their well-being and needs.
• Maintain accurate and confidential client records and report any concerns to the relevant supervisor.

Requirements

• Previous experience in a support worker role or similar caring profession is preferred.
• Excellent communication and interpersonal skills, with empathy and patience.
• Ability to work independently and manage time effectively.
• Understanding of safeguarding procedures and confidentiality.
• Flexibility to work a hybrid schedule and travel within the Stoke-on-Trent area.
