About the Role

Our client is seeking a caring and dependable Outreach Worker to provide essential support to elderly individuals in their homes and communities in Nottingham . This role is focused on enhancing the quality of life for seniors by assisting with daily tasks, providing companionship, and ensuring they have access to necessary services and social connections. You will be a vital link between our clients and the support they need, promoting independence, well-being, and a sense of security for older adults in the community.

Key Responsibilities

• Visit elderly clients in their homes to provide practical assistance and companionship.
• Assist with light household tasks, meal preparation, and running errands.
• Help clients attend appointments and engage in social activities.
• Monitor clients' well-being and report any concerns to the appropriate supervisor.
• Provide information and signposting to other relevant community services and resources.
• Build trusting relationships with clients and their families, ensuring respectful and sensitive care.

Requirements

• Previous experience in a care, support, or outreach role, preferably with the elderly.
• A compassionate, patient, and reliable nature.
• Good communication and listening skills.
• Ability to work independently and manage time effectively.
• Understanding of the challenges faced by older adults.
• DBS check will be required.
