PCN Clinical Pharmacist - Medicines Management & Reviews

The Primary Care Network (PCN) Clinical Pharmacist will support East Lindsey GP Practices by implementing effective medicine management strategies, identifying areas for improvement, and driving change as part of the NHS England Clinical Pharmacists in General Practice pilot.

Patient Services include managing repeat prescriptions, reviewing discharge and clinic letters, addressing urgent requests, and conducting face-to-face reviews for complex regimes.
