PCN Clinical Pharmacist

Job Summary

An exciting opportunity has arisen within Primary Care to work as a Clinical Pharmacist with an Independent Prescriber qualification in our Churchill, Canbury, Orchard and Berrylands Primary Care Network (CCOB PCN). We care for approximately 50,000 patients in four practices across the borough of Kingston; this role would be based at The Churchill Medical Centre, The Orchard Practice, Canbury Medical Centre and Berrylands Surgery.

Job Responsibilities

• Medication reviews to proactively manage patients with complex poly-pharmacy
• Responsibility for areas of chronic disease management, dependent on experience
• Provide support on prescription and medication queries, providing clinical guidance to the pharmacy technicians and prescribing clerks
• Provide clinical leadership on medicines quality improvement
• Contribute to managing some aspects of the quality and outcomes framework
• Work with Kingston CCG pharmacists on the local medicines optimisation framework
• Demonstrate self‑motivation and passion to deliver high quality patient care within general practice

Person Specification

• At least two years post-registration experience
• Degree level qualification
• Registration with the GPhC
• Professional Indemnity membership
• Sound clinical knowledge
• Computer literacy
• Excellent organisational, interpersonal and communication skills
• Desirable: Experience of working in primary care
