We are seeking an experienced and motivated Community Matron to support the delivery of the Enhanced Proactive Care Programme across West Essex for a 12 month FTC. Working within a designated Primary Care Network (PCN), you will provide advanced clinical leadership for patients with complex health needs, frailty, and long-term conditions, helping them remain independent and receive care closer to home.

You will work collaboratively across primary care, community, acute, and voluntary sector services to identify and manage patients at risk of deterioration, delivering personalised care, reducing avoidable hospital admissions, and supporting safe hospital discharge. As an autonomous practitioner, you will play a key role in driving integrated neighbourhood working, proactive care, and the Home First approach, ensuring high-quality, person-centred care for local populations.

This is an exciting opportunity to influence service development, support innovative models of care, and improve outcomes for patients across West Essex.

Main duties of the job

The Community Matron will be a key member of the Integrated Neighbourhood Team, working in partnership with the Primary Care Network (PCN) to deliver a whole-systems approach to the management of health and social care needs. The role will focus on supporting individuals with complex, long-term conditions, whether they are living at home or within a care home setting.
