About the Role
Our client is seeking a proactive and caring Reablement Support Worker to join their team in Reading . You will work in people's homes, providing tailored support to enable them to develop or re-learn essential daily living skills. Key Responsibilities
- Assess individuals' needs and collaboratively develop personalised reablement plans.
- Provide practical support and encouragement to help individuals regain independence in areas such as personal care, mobility, and household tasks.
- Empower clients by teaching and reinforcing skills, fostering confidence and self-reliance.
- Monitor progress, documenting achievements and any challenges encountered, and reporting back to the care team.
- Liaise with families, healthcare professionals, and other agencies to ensure a coordinated approach to care.
- Adapt support strategies based on individual progress and changing needs.
Requirements
- Previous experience in a caregiving role, preferably with an understanding of reablement principles.
- Excellent interpersonal and communication skills, with the ability to motivate and inspire.
- Patience, empathy, and a positive attitude towards helping others regain independence.
- Ability to work autonomously and manage time effectively within client homes across the Reading area.
- A commitment to person-centred care and promoting dignity.
- Must hold a full UK driving licence and have access to a vehicle.
