Job Description
We are recruiting for an experienced Clinical Case Manager to join a specialist community-based complex care service in Exeter.
This is an excellent opportunity for a Registered Nurse with strong community complex care experience who can provide clinical leadership, support care packages and work closely with clients, families, staff and external professionals.
Key Responsibilities
* Assess clients' physical and mental health needs.
* Develop personalised, outcome-focused care plans.
* Complete clinical and environmental risk assessments.
* Review care packages in response to changing needs.
* Provide clinical support to clients and their families.
* Act as a key communication link between families, professionals and care teams.
* Support the identification and induction of staff into complex care packages.
* Provide ongoing clinical support to nurses and care staff.
* Complete staff supervisions, appraisals and competencies.
* Support training and development of clinical and non-clinical staff.
* Interview healthcare staff.
* Work with commissioners to support new care packages.
* Help reduce unplanned hospital admissions through effective case management.
* Ensure care delivery meets CQC and safeguarding requirements.
* Maintain accurate clinical records and electronic care management systems.
* Support Care Coordinators to ensure continuity of care.
* Deal with complaints and concerns appropriately.
* Participate in an out-of-hours on-call rota.
About You
* Registered Nurse with current NMC PIN registration.
* Minimum 2 years' post-qualification experience.
* Minimum 5 years' experience within health and social care.
* Experience within community-based complex care.
* Experience managing and supervising nurses or care teams.
* Strong knowledge of health and social care legislation and CQC requirements.
* Excellent communication and relationship-building skills.
* Highly organised with strong clinical leadership skills.
* Full driving licence and access to your own vehicle.
* Confident working with clients, families and multidisciplinary professionals
