About the Role
Our client is looking for a highly organised and empathetic Remote Care Coordinator to join their expanding team, serving clients across the UK from a remote setting. In this crucial role, you will be the primary point of contact for individuals accessing support services, coordinating their care pathways, and ensuring seamless communication between clients, families, and external agencies. You will play an instrumental part in enhancing the client experience by providing exceptional administrative and logistical support, ensuring that individuals receive the right care at the right time, all managed through efficient remote operations.
Key Responsibilities
- Coordinate and schedule a variety of care services for clients, ensuring efficient resource allocation.
- Act as the main point of contact for clients, their families, and support networks, addressing queries and concerns promptly.
- Maintain accurate and up-to-date client records within the CRM system.
- Liaise with healthcare professionals and external agencies to facilitate holistic care planning.
- Prepare reports on service delivery, client feedback, and operational efficiency.
- Manage client intake processes and ensure all necessary documentation is completed.
Requirements
- Previous experience in care coordination, social work, or a related administrative role within the health and social care sector.
- Excellent organisational and time-management skills, with the ability to multitask effectively.
- Strong IT proficiency, including experience with CRM systems and Microsoft Office Suite.
- Exceptional communication and interpersonal skills, with a compassionate and professional demeanour.
- Ability to work autonomously and proactively in a remote work environment.
