Key Responsibilities

• Act as the primary point of contact for clients, providing information and support regarding care services.
• Assess client needs and develop personalized care navigation plans to facilitate access to relevant resources.

• Educate clients and their families about available services and their rights.
• Maintain accurate and up-to-date client records in the CRM system.

Requirements

• Proven experience in care coordination, social work, or a similar client support role.
• Excellent communication, listening, and empathy skills, with the ability to connect remotely.
• Strong organizational skills and the ability to manage multiple cases simultaneously.
