Key Responsibilities

- Assist clients in scheduling appointments, understanding treatment plans, and navigating insurance or financial assistance options.
- Connect clients with community-based resources such as mental health services, social support groups, and preventative health initiatives.
- Maintain accurate and confidential client records within a secure electronic health record system .
- Educate clients on health promotion, disease prevention, and healthy lifestyle choices through remote communication channels.
Requirements
- Experience in a healthcare , public health, social work, or community support role.
- Strong understanding of healthcare systems, patient pathways, and community resources.
- Excellent communication, empathy, and active listening skills, adapted for remote interaction.
- Proficiency in using telehealth platforms, virtual meeting software, and digital communication tools.
- Ability to manage a caseload effectively and advocate for client needs.
- A dedicated workspace with reliable high-speed internet access is mandatory for this remote position, serving clients across the UK .
