About the Role

Our client is seeking a proactive and dedicated Remote Community Outreach Worker to join their dynamic, fully remote team, serving individuals in need across the Plymouth region and beyond. This role involves connecting with vulnerable populations, assessing their needs, and coordinating access to vital resources and support services, all managed through remote communication channels. You will be a key facilitator, ensuring individuals receive the assistance they require to improve their well-being and independence, working from the convenience of your home office. Key Responsibilities

• Conduct remote needs assessments with individuals experiencing various challenges, identifying appropriate support services and resources.
• Coordinate and facilitate access to community resources, including housing support, healthcare services, employment assistance, and social programs.
• Provide information, guidance, and emotional support to clients via phone, video calls, and digital platforms.
• Maintain accurate and confidential client records, tracking progress and ensuring all interactions are well-documented in the CRM system.
• Liaise with partner organizations, charities, and statutory bodies to build strong referral pathways and collaborative support networks.
• Develop and implement outreach strategies to engage with hard-to-reach populations and raise awareness of available services.

Requirements

• Previous experience in social work, community development, outreach, or a related field.
• Excellent communication and interpersonal skills, with the ability to connect with individuals from diverse backgrounds via remote channels.
• Proficiency in using virtual communication tools (e.g., Zoom, Teams) and digital record-keeping systems.
• Strong organizational and time-management skills to effectively manage a caseload remotely.
• Ability to work autonomously and demonstrate initiative in identifying and addressing client needs.
