About the Role
Our client is seeking a highly organized and empathetic Remote Community Support Specialist to join their fully remote team, serving individuals in need across the UK, with a focus on the Oxfordshire area. In this role, you will be instrumental in connecting individuals with essential community resources, providing guidance on accessing support services, and advocating for their needs through virtual means. You will leverage digital platforms, phone, and video conferencing to offer support, coordinate services, and ensure clients receive the assistance they require without geographical limitations. Key Responsibilities
- Conduct needs assessments and develop personalized support pathways for clients via remote communication channels.
- Research and identify relevant community resources, social services, and support groups for clients based on their specific needs.
- Assist clients in navigating application processes for benefits, housing, healthcare, and other essential services.
- Provide ongoing remote guidance and encouragement to clients as they work towards their personal goals.
- Maintain accurate and detailed case notes on the client management system, ensuring confidentiality and compliance.
- Liaise with partner agencies and service providers virtually to facilitate client referrals and coordinate support efforts.
Requirements
- Proven experience in a community support , social work, case management, or similar role.
- Strong understanding of social care principles, available support services, and the challenges faced by vulnerable individuals.
- Excellent remote communication skills, including active listening, empathy, and the ability to build rapport virtually.
- Proficiency in using video conferencing software, digital CRMs, and online collaborative tools.
- Ability to work autonomously, manage a caseload effectively, and meet deadlines in a remote setting.
- A dedicated workspace with reliable high-speed internet access is mandatory.
