About the Role

Our client is seeking a dedicated and licensed Pharmacist to provide remote clinical services, operating from anywhere within the UK. Key Responsibilities

• Conduct medication therapy management (MTM) services remotely, including medication reviews and reconciliation.
• Provide patient counseling and education on medication adherence, potential side effects, and lifestyle modifications.
• Collaborate with physicians and other healthcare professionals to optimize patient treatment plans and manage chronic conditions.
• Review patient electronic health records to identify potential drug-related problems and recommend interventions.
• Ensure compliance with all relevant pharmacy laws, regulations, and company policies.
• Document all patient interactions and clinical recommendations accurately and promptly in the patient management system.

Requirements

• Registered Pharmacist (GPhC) with a valid license to practice in the UK.
• Proven experience as a practicing pharmacist, with experience in clinical pharmacy or MTM services being a strong advantage.
• Excellent knowledge of pharmacology, drug interactions, and therapeutic guidelines.
• Strong communication, interpersonal, and patient counseling skills.
• Proficiency with telehealth platforms and electronic health record systems.
• Ability to work independently, manage time effectively, and maintain a high level of professionalism in a remote setting.
