About the Role

Our client is looking for a proactive and organised Remote Social Care Coordinator to manage and deliver support services from a distance. This role is crucial for ensuring that clients receive seamless and effective care planning and coordination, irrespective of their location. You will be a key point of contact, liaising with clients, families, and external agencies to ensure comprehensive care packages are implemented and reviewed. Key Responsibilities

• Coordinate and manage individualised care plans for clients, ensuring they are met effectively and efficiently.
• Liaise regularly with clients, families, and a network of care providers via phone, email, and video conferencing.
• Assess client needs and facilitate referrals to appropriate services and resources.
• Maintain accurate and up-to-date client records on our digital platform.
• Monitor the quality of care being delivered and address any issues or concerns raised.
• Support the recruitment, training, and ongoing supervision of remote care staff.

Requirements

• Proven experience in social care coordination, case management, or a similar role.
