About the Role
Our client is looking for a compassionate and organised Remote Support Coordinator to work with vulnerable adults, based entirely from home and serving the Cardiff region. You will play a critical role in ensuring individuals receive the support they need to maintain their independence and quality of life, managing their care plans and connecting them with essential services via digital channels. This position requires a proactive approach to communication and a strong ability to coordinate a network of support without direct physical interaction. Key Responsibilities
- Manage caseloads of vulnerable adults, coordinating their support packages remotely.
- Conduct initial and ongoing needs assessments via telephone and video conferencing.
- Liaise with family members, carers, and healthcare professionals to ensure comprehensive care.
- Facilitate access to relevant social, health, and well-being services.
- Maintain accurate electronic records and client case notes.
- Respond promptly to client inquiries and escalating concerns as necessary.
Requirements
- Previous experience in a support work or care coordination role.
- Excellent communication skills, with a warm and empathetic telephone and video presence.
- Proficiency in using digital tools for communication and case management.
- Knowledge of safeguarding procedures for vulnerable adults.
- Ability to work independently and manage time effectively in a remote setting.
- Understanding of the challenges faced by vulnerable adults in the Cardiff area.
