Key Responsibilities

- Develop, review, and update client support plans in line with regulatory requirements and individual needs, liaising with healthcare professionals as necessary.
- Monitor the quality of service delivery, conducting regular checks and implementing improvement strategies to maintain high standards.
- Maintain accurate and up-to-date client records, service logs, and staff rotas using the company's digital management system.
- Facilitate communication between the support team, management, and other stakeholders, ensuring information flows smoothly and effectively.
Requirements
- Proven experience in a coordination, supervisory, or management role within the social care or community services sector.
- Strong understanding of care planning, client needs assessment, and regulatory compliance in the UK.
- Excellent organisational skills, with the ability to manage multiple tasks and priorities in a fast-paced environment.
- Exceptional communication and interpersonal skills, capable of building strong relationships remotely.
- Proficiency in using scheduling software, CRM systems, and other digital tools for remote work.
