Senior Community Nurse | City Health Care Partnership CIC

You’ll support people needing rehabilitation in the community, helping them recover, stay independent and improve their quality of life.

You’ll work as part of a multidisciplinary team, providing high-quality, nurse-led care to patients with a range of complex needs. This senior role combines hands‑on clinical practice with leadership, helping shape safe and effective services. You’ll build strong relationships with patients, carers and partners, ensuring care is joined‑up and person‑centered

What you’ll do:

• Assess, diagnose and manage patient care, including treatment, referral and discharge planning
• Coordinate and prioritise a caseload, supporting timely rehabilitation and recovery
• Work closely with multidisciplinary teams to plan and deliver joined‑up care
• Provide guidance, supervision and support to junior staff and students
• Contribute to service development, quality improvement and evidence‑based practice

What we’re looking for:

• A registered nurse with professional registration and up‑to‑date clinical knowledge
• Experience of working in multidisciplinary teams and managing complex patient needs
• Confidence to work independently and make clinical decisions
• Strong communication and organisational skills, with a focus on patient‑centred care
• A commitment to learning, development and supporting others to grow
