Our client is seeking an experienced and compassionate Senior Community Support Worker to join their dedicated team. Key Responsibilities

• Conduct remote needs assessments and develop personalized support plans for clients.
• Provide emotional support, guidance, and practical assistance to individuals facing challenges.
• Coordinate with families, healthcare professionals, and other support services to ensure comprehensive care.
• Facilitate remote social inclusion activities and skill-building sessions for clients.
• Mentor and support junior community support workers, providing guidance and feedback on their cases.
• Maintain accurate and confidential client records using digital systems.

Requirements

• Proven experience in a community support or social care role, with at least 2 years in a senior or lead capacity.
• Excellent communication and interpersonal skills, with the ability to build rapport remotely.
• Strong understanding of safeguarding principles and best practices in social care.
• Proficiency in using digital tools for case management, virtual communication, and record-keeping.
• Ability to work autonomously and manage a caseload effectively from a remote setting.
• Relevant qualifications such as NVQ Level 3 or 4 in Health and Social Care, or equivalent.
