Senior Diabetes Matron: MDT Leader, System-Wide

South and East Leeds General Practice Group is recruiting a Diabetes Matron to lead senior nursing strategy across a multi-neighbourhood footprint in Leeds. The role focuses on advanced clinical decision-making, MDT leadership, and workforce development, interfacing with Tier 2/4 services and local GP networks.

The post is 0.4 WTE (15 hours per week) with a flexible pattern and opportunities to shape the Tier 3 nursing offer.
