Senior District Nurse – Community Care Lead (Flexible)

South Warwickshire NHS Foundation Trust is seeking a Band 6 District Nurse/Case Manager for the Placed Based Team 3 in Nuneaton Central. You will assess and manage health needs of housebound patients at home, with a focus on prevention and reduced hospital admissions.

The role requires leadership, excellent communication, and the ability to manage a caseload across North Warwickshire. Driving licence and travel are essential; seven days work and inductions are provided.
