Key Responsibilities

• Conduct comprehensive assessments of individual needs, developing and coordinating person-centred care and support plans.
• Provide specialist advice, support, and intervention to individuals, families, and carers facing complex social issues.
• Facilitate multi-agency working, ensuring effective collaboration and information sharing to achieve optimal outcomes for clients.
• Mentor and support junior members of the social work team, offering guidance on casework and professional development.
• Maintain accurate and up-to-date case records, adhering to all professional standards and organisational policies, accessible through secure remote platforms.
• Champion best practice and contribute to the continuous improvement of services offered by our remote team.

Requirements

• Recognised social work qualification (e.g., BA/BSc in Social Work, DipSW) and current professional registration (e.g., Social Work England).
• Significant post-qualifying experience in a statutory or voluntary sector social work setting, preferably with experience in managing complex caseloads.
• Demonstrated ability to conduct thorough assessments, formulate care plans, and review their effectiveness.
• Excellent communication, negotiation, and interpersonal skills, with the ability to engage effectively with diverse client groups and professionals remotely.
• Proficiency in using digital case management systems and other relevant IT tools for remote working.
