About the Role
Our client is seeking a highly organised and empathetic Senior Support Coordinator to join their expanding team, operating on a fully remote basis to serve individuals across Northern Ireland . This vital role involves coordinating comprehensive support services for clients with complex needs, ensuring seamless delivery and proactive management of their care packages. You will be a key point of contact, working closely with clients, their families, and a network of external agencies to facilitate positive outcomes and promote independence. This position offers the flexibility of remote work, allowing you to contribute from anywhere within Northern Ireland while making a substantial difference.
Key Responsibilities
- Coordinate and manage a caseload of clients, developing and reviewing personalised support plans.
- Liaise with a range of health and social care professionals, statutory bodies, and third-sector organisations.
- Conduct thorough needs assessments and risk assessments to ensure appropriate support is in place.
- Monitor the effectiveness of support services, identifying areas for improvement and implementing necessary changes.
- Provide guidance and support to junior members of the coordination team.
- Maintain accurate and up-to-date client records using digital systems.
Requirements
- Significant experience in social care, disability services, or a related field, with at least two years in a coordination or supervisory role.
- A deep understanding of the challenges faced by individuals with complex needs and their families.
- Excellent organisational, planning, and time management skills.
- Strong communication and interpersonal abilities, with the capacity to build rapport remotely.
- Proficiency in using digital case management software and standard office applications.
- A proactive approach to problem-solving and a commitment to continuous service improvement.
