About the Role

Our client is seeking an experienced and meticulous Social Care Assessor to join their team serving the Reading area. This role is responsible for conducting thorough assessments of individuals' needs for social care services, ensuring they receive appropriate support to maintain their independence and quality of life. You will work closely with individuals, families, and other professionals to develop tailored care plans. This hybrid role allows for a balance of on-site assessments and administrative tasks, which can be managed remotely, providing flexibility while ensuring critical support is delivered effectively in Reading .

Key Responsibilities

• Conduct comprehensive needs assessments for individuals requiring social care support.
• Develop, implement, and review personalised care and support plans.
• Liaise with care providers, health services, and other agencies to arrange services.
• Ensure assessments and plans comply with relevant legislation and organisational policies.
• Maintain accurate and up-to-date records on the assessment system.
• Provide information and advice to individuals and their families regarding available services.

Requirements

• Proven experience as a Social Care Assessor or in a similar role.
• Knowledge of relevant social care legislation and best practices.
• Excellent assessment, communication, and report-writing skills.
• Ability to manage a caseload effectively and prioritise tasks.
• Proficiency in using assessment software and standard IT applications.
• A relevant qualification in social work, health and social care, or a related field is essential.
