About the Role
Our client is seeking a meticulous and empathetic Social Care Assessor to join their team in Reading . This role is central to ensuring that individuals requiring social care support receive appropriate assessments and that effective support plans are put in place. You will work with adults and their families to understand their needs, evaluate available resources, and facilitate access to services that promote independence and well-being. Key Responsibilities
- Conduct comprehensive and accurate social care needs assessments for adults, in line with legislative frameworks and organisational policies.
- Develop, review, and monitor personalised support plans, ensuring they meet the identified needs and preferences of individuals.
- Assess the eligibility of individuals for social care services and advise them on available options.
- Liaise effectively with service users, their families, and a range of internal and external agencies to coordinate care.
- Maintain detailed and confidential records of assessments, plans, and ongoing case management.
- Identify potential risks and safeguarding concerns, escalating them appropriately according to established procedures.
Requirements
- Proven experience in social care assessment or a related social work role.
- Thorough understanding of relevant legislation, policies, and procedures governing adult social care.
- Excellent assessment, analytical, and problem-solving skills.
- Strong communication and interpersonal skills, with the ability to engage sensitively with clients.
- Proficiency in IT systems for case management and record-keeping.
- A professional qualification in Social Work, Occupational Therapy, or Health and Social Care is required.
