Social Care Officer

About the Role

Our client is recruiting for a proactive and skilled Social Care Officer to contribute to essential community services in Newcastle upon Tyne . This role involves supporting individuals and families facing various challenges, including those with learning disabilities, mental health issues, or complex social needs. You will play a key part in assessing needs, developing support plans, and coordinating with a network of agencies to ensure clients receive appropriate and timely assistance. The position offers the chance to work in a dynamic environment, fostering independence and improving the quality of life for residents across Tyne and Wear , contributing to a more inclusive and supportive local community structure.

Key Responsibilities

• Conduct comprehensive needs assessments for individuals and families within the community.
• Develop, implement, and review personalized support plans in collaboration with clients.
• Provide direct support and advocacy for clients navigating complex systems and services.
• Liaise effectively with statutory and voluntary sector organisations to secure appropriate resources.
• Maintain accurate and confidential client records using designated case management systems.
• Respond to safeguarding concerns and escalate appropriately according to established procedures.

Requirements

• NVQ Level 3 or equivalent qualification in Health and Social Care is preferred.
• Demonstrable experience working within a social care setting.
• Strong understanding of relevant legislation, policies, and best practices in social care.
• Excellent assessment, planning, and review skills.
• Proficiency in IT, including Microsoft Office and social care databases.
• Ability to travel independently across the Newcastle upon Tyne region.
