About the Role

Key Responsibilities

• Conduct assessments of individual needs and develop tailored support plans.
• Provide direct support and assistance to clients in managing their daily lives and achieving personal outcomes.
• Liaise with external agencies and services to ensure coordinated and comprehensive support.
• Maintain accurate and confidential client records, documenting progress and interventions.
• Offer emotional support and guidance, promoting independence and well-being.
• Participate in team meetings, case reviews, and ongoing training to enhance service delivery.

Requirements

• Experience working in social care or a related field is essential.
• NVQ Level 3 or equivalent qualification in Health and Social Care is preferred.
• Strong understanding of safeguarding vulnerable adults and children.
• Excellent communication, negotiation, and problem-solving skills.
• Ability to work flexibly, including some evening and weekend work as required.
• Proficiency in using case management software and standard office applications.
