About the Role

Our client is looking for a proactive and dedicated Social Care Officer to join their Community & Social Care team in Sunderland . This role involves providing direct support to individuals and families facing various challenges, aiming to improve their quality of life and promote independence. You will work across both community settings and potentially from a local office or remotely, assessing needs, developing support plans, and coordinating services. Key Responsibilities

• Conduct needs assessments for individuals and families, identifying social care requirements and risks.
• Develop, implement, and review tailored support plans in collaboration with clients and relevant stakeholders.
• Liaise with a range of statutory and voluntary agencies to coordinate the provision of services and resources.
• Provide practical advice, information, and emotional support to clients, empowering them to manage their own lives.
• Maintain accurate and confidential client records and case notes, adhering to organizational policies and data protection regulations.
• Participate in team meetings, case conferences, and training sessions to enhance professional development and service delivery.

Requirements

• Experience in a social care or related field, with a good understanding of social policy and welfare services.
• Strong assessment, planning, and review skills, with the ability to manage a caseload effectively.
• Excellent communication, negotiation, and interpersonal skills.
• Proficiency in using IT systems for record-keeping and communication.
• A commitment to promoting equality, diversity, and inclusion in all aspects of work.
• Relevant qualifications in Social Work, Health and Social Care, or equivalent experience.
